The purpose of this role is to ensure best practice and the maintenance of professional standards in delivering focused support to the individuals using the Outreach Service. Establishing and maintaining links with families, carers, organisations and other professionals to ensure positive outcomes are achieved.
Responsibilities and Duties
To cooperate and engage effectively with Colleagues whilst taking responsibility for self.
To utilise information technology to effectively support the Users of the Service.
Use a Client database to facilitate recording of outcomes and other key information
Use various Microsoft packages required to deliver a range of Service user documentation
Ensure that all records are kept to a high standard
