Apprentice Community Support Worker

THE ROLE

The purpose of this role is to ensure best practice and the maintenance of professional standards in delivering focused support to the individuals using the Outreach Service. Establishing and maintaining links with families, carers, organisations and other professionals to ensure positive outcomes are achieved.

 

Responsibilities and Duties

  • To support Service Users to reach agreed outcomes.
  • Direct support with Individuals to engage in agreed opportunities, acting as a keyworker
  • Create and implement risk assessments
  • Ensuring that support is delivered in a Person Centred Manner.
  • To cooperate and engage effectively with Colleagues whilst taking responsibility for self.

  • To utilise information technology to effectively support the Users of the Service.

  • Use a Client database to facilitate recording of outcomes and other key information

  • Use various Microsoft packages required to deliver a range of Service user documentation

  • Ensure that all records are kept to a high standard

 

THE CANDIDATE

  • To hold an NVQ 2 in Care/Support (or above) or other relevant qualification
  • To have current knowledge of current issues related to the provision of services to people with learning disabilities and Autism. 
  • To demonstrate an understanding of positive approaches in managing unpredictable behaviour that may cause harm to self or others
  • Ability to devise and implement opportunities and activities promoting independence, choice and achieving identified outcomes
  • Demonstrable experience of supporting adults with learning disabilities and Autism with complex needs who may present unpredictable behaviour that may cause harm to self or others.
  • Experience of working as part of a team.
  • Experience of organising and delivering social and learning opportunities for adults with learning disabilities and Autism.
  • Experience of delivering opportunities in the community for adults with learning disabilities and Autism. 

OTHER INFORMATION

As well as becoming qualified in your chosen area, you will receive a London Living Wage salary of £19,750 per annum, which is one of the best apprenticeship salaries in London!
